South Dakotans can register today to be an organ and tissue donor by checking the box on their driver’s license or state ID card application or renewal form, or by printing and mailing the organ and tissue registry form below.
Download the organ and tissue donation registry form and return the completed form to the address here. There is no charge to add ‘donor’ to your driver’s license between renewals. You will not receive a new license, but your designation will be added to the state database.
