Minnesota
Minnesotans can register online to become an organ and tissue donor through Donate Life Minnesota. If you have any questions or would prefer to enroll by mail, please contact LifeSource at 1.888.5.DONATE.
North Dakota
North Dakotans can register online to become an organ and tissue donor through the North Dakota Department of Transportation. If you have any questions or concerns about registering to be a donor online, please contact the Department of Transportation by calling (701) 328-2600, email dl@nd.gov or visit your local driver’s license station.
South Dakota
South Dakotans can register today to be an organ and tissue donor by checking the box on their driver’s license or state ID card application or renewal form, or by printing and mailing the organ and tissue donation registry form. Completed forms should be returned to the address below. There is no charge to add ‘donor’ to your driver’s license in between renewals. You will not receive a new license, but your designation will be added to the State database and you will receive a sticker for your license.
Wisconsin
Wisconsin residents can register online to become an organ and tissue donor through the Donate Life Wisconsin website.
